To participate in the ARM Program, submit a signed confidentiality form [Click Here] and a brief (one page) information sheet documenting some demographics of the institution and providing essential information about data [Click Here].

 

• Complete the institution information form on-line and print
• Print, complete and sign the confidentiality form
• Fax these to the ARM Program at 352-392-7766. 

 

Please read the data specifications [Click Here]. We ask for a minimum of three years of susceptibility data, preferably annualized. More than three years is acceptable.  If available, we also accept antibiotic utilization data. Data can be submitted electronically to jgums@ufl.edu or mailed to:

We provide a customized report discussing the patterns of resistance with benchmarks to regional, state, or national levels. We also have the capacity to deliver educational programs that are approved for CME and ACPE credit to the professional staff at your hospital.
